I want to be clear and transparent about who I am and how I work, so you can decide whether this feels like a good fit.

I, Jonathan Stein, am a Transformational Holistic Life Coach with a master’s degree in Counseling and Psychology and training in transpersonal psychotherapy, drama therapy, holistic life coaching, and body-oriented approaches. I’m not a state-licensed medical or mental health provider. I don’t diagnose or treat mental health conditions, prescribe medication, or provide medical care or psychotherapy.

Our work together is meant to support personal growth, clearer understanding, and practical change in everyday life. It isn’t a substitute for medical, psychiatric, or licensed mental health care. If you have a medical or mental health concern, I encourage you to seek support from a licensed provider. If you’re in immediate danger or feel unsafe, call 911 or your local emergency services right away.

By signing this form, you’re confirming that you understand the scope of my role, that outcomes vary, and that you’re choosing to participate with care and personal responsibility.

My Role and Training

My work is shaped by more than 30 years of experience in counseling, therapeutic settings, and private practice. That background informs how I listen, the patterns I notice, and the care I bring to our work. The service I provide today, however, is coaching, and I use the professional title Transformational Holistic Life Coach.

Nature of My Services and Online Practice

My sessions may include practical reflection, education, guidance, and experiential support. I don’t provide medical, nutritional, or medication advice.

My practice is based in Massachusetts, and all sessions are currently offered remotely by video or phone. The service I provide is coaching, not telehealth, teletherapy, or distance mental health treatment.

Because laws and professional rules can vary by location, I’ll ask where you’ll be physically located during sessions and may need to confirm whether I can appropriately work with you there.

If it becomes clear that you need support outside the scope of coaching, we’ll talk about that directly. I may encourage you to seek care from an appropriately licensed provider in your area.

Safety and Crisis Support

I don’t provide crisis services or monitor messages continuously.

Responsibility and Expectations

Coaching is a collaborative process. I’ll bring care, honesty, attention, and experience to our work. You’re responsible for deciding what you share, how you understand and use what we discuss, and what choices you make.

I may offer questions, observations, practices, or different ways of looking at a situation. You’re always free to consider them, question them, or decide they aren’t right for you.

Limitation of Liability

To the fullest extent permitted by law, I won’t be liable for decisions you make, actions you take, or outcomes that result from how you interpret or apply coaching services or related materials.

Nothing in this Disclaimer waives any right or limits any responsibility that cannot legally be waived or limited.

No Guarantees

I can’t promise a particular outcome. Coaching supports reflection, learning, and practical change, but results vary from person to person.

Electronic Signature Consent

By typing your name and submitting this form, you confirm that you’ve read and agree to this Disclaimer. You agree to sign it electronically and intend your typed name to serve as your signature.

Governing Law and Jurisdiction

To the extent permitted by law, this Disclaimer and any dispute arising from the coaching services described here will be governed by the laws of the Commonwealth of Massachusetts. Any legal proceeding will be brought in a state court located in Hampshire County, Massachusetts, or in the federal court serving Hampshire County.

Relationship to Other Agreements

This Disclaimer addresses the scope of my services and the responsibilities described here. It works alongside any separate coaching agreement, office policies, consent form, or privacy notice that applies to our work. If any part of this Disclaimer is found unenforceable, the remaining provisions will stay in effect.
